not quite sure how to explain myself with this one. this is a victim of a post apocalyptic world in which toxins in the air melt off your skin, in order to preserve whatever is left of one's beauty they must staple any remaining flesh they have to their head because beauty depends on your skin....so yeah. the redness is bloody exposed muscle
